The MINOR PATIENT CONSENT FORM is for those children under the age of 18 who are unable to be accompanied by their parents at their first visit or any subsequent visit.

Do you take my insurance?
Paradise Dental is currently accepting new patients with Delta Dental and Healthcare Sarasota dental plans. For those that have one of these two insurance plans, we will confirm eligibility and benefits and we ask only that you pay your estimated co-payment at the time of service. We will send your claim and accept the payment. Sometimes insurance company's guidelines are not accurate for our local area and small differences in our estimated co-payments can occur. When this happens, you will be billed or credited for the difference.
Can I go to Paradise Dental if you don't participate in my insurance plan?
If you have insurance with a company we don't participate with, we will be happy to determine what benefit you will be eligible to receive as a patient at Paradise Dental. You will be responsible for your charges at the time of service, then we will send in your claims so you can receive reimbursement. Many times it is the paperwork that discourages patients from using a "non-participating" dentist. At Paradise Dental, we want to encourage patients to see the dentist of their choice and will take care of your claim paperwork to make this option easier.
